The First Step to Healthier You

- We will need a copy of your insurance cards and photoID. You can email them to us at office@bmisurgery.org or fax to 815-717-8339
- We will need you to complete the Health History Form.
Once we have verified your insurance and received your health history form we will call you to schedule a consultation with one of our Nurse Practitioners who specialize in weight loss options specific to your needs.
