New Patients

Once you have attended an informational seminar and scheduled your first consultation with our physician, please download the BMI Patient Portal software from Raintree Systems by clicking the blue button on the right.

Once this application has been saved on your local computer, please double click the file to open and run. Answer the questions, then submit your information so that we will have the important medical information we need for your first consultation.

Established Patients

If you are an established patient with BMI Surgery, you can now take advantage of BMI Surgery’s secure online Secure Patient Portal. On your first visit, just complete the quick and easy registration so you will be able to:

Request New Appointments
View and Update Personal Demographics
View and Print Forms

The Patient Portal also gives the staff at BMI Surgery a secure and efficient method of providing you with answers and access to personal information, improving the efficiency of our staff in the office.

Of course, the Secure Patient Portal is available to you 24 hours a day, 7 days a week, 365 days per year.

The Secure Patient Portal is not intended to replace an office visit. It should only be used for routine and non-urgent matters. If this is a Medical Emergency please immediately call 911. For urgent medical matters managed by our office, please contact us during office hours.
